Leeds Community Healthcare NHS Trust is seeking dedicated Specialist Public Health Nurses (Health Visitors) for their 0-19 Public Health Integrated Nursing Service. As a member of the 0-19 teams, you will engage in initiatives aimed at supporting children, young people, and their families in Leeds, contributing to improved health outcomes.
Main Responsibilities
- Promote and enhance health outcomes by identifying and prioritizing interventions based on individual needs.
- Facilitate Preparation for Birth and Beyond groups, both virtually and in person.
- Provide specialist breastfeeding support and work within the Clinical Triage Team.
- Develop digital platforms to improve engagement with families.
- Implement evidence-based interventions for various populations including young parents and young carers.
- Engage with service design involving parents and young people.
- Collaborate with Children’s Centres, Healthy Schools, and other stakeholders.
