Folkestone, Hythe and Rural Primary Care Network is looking for an experienced Advanced Nurse Practitioner to join its Care Home and Integrated Neighbourhood Team. This is a chance to shape how the PCN supports care home residents and patients in their own homes, working alongside GP practices and partner agencies to deliver safe, person-centred care.
Your responsibilities
- Carry out initial assessments and ongoing reviews of care home and home-based residents, keeping care plans up to date
- Triage patients and deliver appropriate treatment by phone and during care home or home visits
- Examine, assess and diagnose patients, managing their clinical care within your scope of practice
- Coordinate with the wider multi-disciplinary team to meet individual patient needs across nursing, residential and home settings
- Support safeguarding processes and contribute to audits, significant event reviews and root cause analysis
Your profile
- At least two years of post-graduate experience, including work in care homes or community settings
- Registered General Nurse with NMC registration and a Master's level qualification in Advanced Clinical Practice
- Independent Prescriber qualification and evidence of ongoing continuing professional development
- Strong consultation, diagnostic and clinical decision-making skills
- Confident communicator able to lead, supervise and collaborate within a multi-disciplinary team
- Full UK driving licence and access to a car
