Folkestone, Hythe and Rural PCN is looking for a motivated Social Prescriber / Health and Well Being Coach to join its friendly primary care network across the South Kent Coast. This role sits at the heart of an Integrated Neighbourhood Team, helping frail and vulnerable patients stay safe, well and independent at home while bridging the gap between GP practices, multidisciplinary teams and other community services.

Your responsibilities

  • Build trusting relationships with patients, focusing on what matters most to them and supporting self-management
  • Identify and support patients through risk stratification, referrals and frequent A&E attendance data
  • Coordinate with GP practices, community teams, voluntary services and MDT meetings to ensure joined-up support
  • Provide telephone cover, appointment booking and message handling when required
  • Manage assigned projects and contribute ideas to improve service delivery

Your profile

  • Good understanding of primary care, GP practices or the wider NHS system
  • Experience working within a multidisciplinary team environment
  • Strong administrative skills with excellent attention to detail
  • Confident using Microsoft Office applications such as Word, PowerPoint and Outlook
  • Ability to manage varying workloads and communicate clearly, both orally and in writing
  • GCSEs in English and Maths (or equivalent) and an NVQ Level 3 or equivalent administrative qualification
