Phyllis Tuckwell Hospice Care is looking for compassionate Health Care Assistants to join its Hospice at Home team, delivering end-of-life support to patients in their own homes across Farnham and the Guildford area. This is a chance to make a genuine difference for people facing advanced or terminal illness, working closely with a multidisciplinary team of registered nurses and other professionals.

Your responsibilities

  • Support qualified nursing staff in delivering hands-on care to patients with palliative and end-of-life needs
  • Assist with assessment, planning and delivery of care within the scope of your training
  • Provide comfort, information and reassurance to patients, families and carers in their own homes
  • Report and escalate any clinical or safety concerns promptly to the nurse in charge
  • Keep accurate and up-to-date patient records in line with organisational standards

Your profile

  • Previous experience of care work in a home, hospital or care setting
  • A recognised care qualification, or willingness to complete the Care Certificate
  • Understanding of caring for people with progressive or terminal illness
  • Strong communication skills and genuine empathy for patients and families
  • Ability to work well within a team while also being proactive and self-motivated
